Notes on Completing the Survey

To complete the survey on-line - just click on the NEXT button below to go to the first question.

You will notice that:

  • There is a Home button which will bring you back to this page again.
  • There is a progress bar which changes colour as you complete the questionnaire which means that you will see how soon you will be finished.
  • Each question has a section heading and a title. When you reach the end of the questionnaire you will be able to revisit any part of the questionnaire to amend your replies, using these headings as navigation aids.
  • Once you have completed the survey, click on the Submit button, which sends the completed questionnaire to our Web server
  • If you suffer a communication problem and need to re-load the page - this will clear all your entries that you have made. You can safely reload the question by right-clicking on the question and selecting re-load or refresh frame.
Please note that you will need Netscape v3 or Microsoft Explorer v4 and above to be able to complete this survey successfully and Javascript needs to be enabled. If you need to download updated versions of a browser, please visit Netscape or Microsoft or Mozilla Firefox. Also, if you wish to use the survey navigator to review your answers, your browser needs to be Java enabled.
